WHO CAN APPLY
Individuals & families in need of medical financial assistance in the towns of North Canaan, Canaan (Falls Village), Cornwall, Goshen, Kent, Norfolk, Sharon, Salisbury/Lakeville and Warren in CT.
GRANT INFORMATION
Applications are accepted on behalf of clients by health & social service agency representatives or clergy and may be submitted by fax, email or mail. Decisions are generally given within 10 working days.
The Blue Horizon Health Fund supports individuals by paying for medical expenses including: therapeutic, chiropractic, naturopathic and surgical care and treatment of indigent, sick and helpless persons regardless of race, color or creed who reside in the towns of North Canaan, Canaan (Falls Village), Cornwall, Goshen, Kent, Norfolk, Sharon, Salisbury/Lakeville and Warren.
ELIGIBILITY
Please be in touch with the social worker in your town, who can submit an application on your behalf. Applicants who have received a Blue Horizon grant are invited to reapply for future funding should they still be in need of medical financial assistance.
AWARDS
Awards are generally up to $750 but may increase depending on financial need.
